The Insurance Information That You Need to Know
Please have your most current insurance cards available at each visit. As PPOs and other Managed Care Insurance programs are becoming more common, it is important that you know how your insurance works and what your responsibilities are under your insurance plan.
If you are a member of a managed care plan, please confirm with your insurance carrier that we are part of your network. We will assist you as much as possible, but we can only work within the limitations of the information you provide. We are required by our contracts with insurers to collect co-payments for services at the time they are delivered. You are responsible for any deductibles, co-insurance, and non-covered services.
Surgical fills will be billed to your insurance carrier. We must have your insurance information in the office before surgery can be scheduled. We will contact your insurance company regarding eligibility and benefits for your surgery. Depending on your deductible balance, a surgery deposit may be required.
If you do not have insurance, payment is due in full before surgery. Please be aware that some companies pay a fixed allowance for certain procedures and pay a percentage of the charge. It is your responsibility to pay any deductible amount, coinsurance, or any other balance left unpaid by your insurance company.

Medicare Information
As Medicare-participating physicians, we can file your Medicare claim for you.
If you have a supplemental insurance plan, please provide our office with a copy of the insurance card. We will assist you in billing your Medicare supplemental insurance claim.

Fox Valley Ear, Nose & Throat Associates accepts cash, personal checks, Visa, MasterCard, Discover, and American Express. We will be happy to assist you in using Care Credit to set up financing.
